RELEASE OF LIABILITY & INDEMNITY AGREEMENT

By signing this form, I acknowledge that I am voluntarily participating in the Harvest Festival
organized by Love Beyond Autism (LBA). I understand that participation involves travel and activities that may
expose me to potential hazards.

I agree to assume full responsibility for any risks, injuries, illness (including potential exposure to communicable
diseases such as COVID-19), or damages that may occur as a result of my participation in this event.

I hereby release, waive, and discharge Love Beyond Autism Foundation Inc., its board members, volunteers, and
affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage,
or injury that may be sustained during this event.

This release is binding upon myself, my heirs, personal representatives, and assigns. I certify that I am 18 years of
age or older and legally competent to sign this agreement.

All sales are final. No refunds will be issued.
